According to research by the university of Kelaniya, there isn’t a reasonable answer for kidney diseases. So, it has been officially named as Chronic kidney disease of unknown cause. However, because of this, the patient’s kidneys gradually deteriorate; this experience impacts the family and the patient over time. 

Chronic Kidney Disease of uncertain etiology (CKDu) first made its presence known in the mid-90s, and since then, it has become a pressing public health concern. As they had mentioned, this silent epidemic affects thousands of individuals in the North Central Province of Sri Lanka. Known as CKD/CKDu(Chronic kidney disease unknown), this debilitating condition has seen a surge in incidence rates over the years, with some areas reporting a four-fold increase from 2009 to 2016. Despite some decline in recent years, the disease continues to afflict a significant proportion of the population, with over 30,000 patients identified in the region.

A closer look at the demographics of CKD/CKDu patients reveals some alarming trends. The disease disproportionately affects older individuals, with more than 70% of patients over 50. Men are also more likely to be affected, and farmers are at the highest risk of developing the disease.

Sadly, the prognosis for CKD/CKDu patients is not promising, with a significant proportion of patients dying within the first few years of diagnosis. Despite this, there is hope for a better future. Efforts are underway to map out the disease and identify areas of high prevalence in the hopes of developing targeted interventions. By shedding light on this silent epidemic, we can work towards a world where CKD/CKDu is no longer a threat to the health and well-being of the people of Sri Lanka.

This insidious disease strikes without warning, affecting individuals with no known underlying cause for Chronic Kidney Disease (CKD). Researchers have found a significant association between CKDu and the rural agricultural population, with males being more commonly affected in both numbers and severity.

Discovering the reasons for the unknown chronic kidney disease in the north-central province of Sri Lanka

The histopathological features of the disease are equally concerning, with a predominant chronic tubulointerstitial nephritis associated with glomerular scarring, tubular atrophy, and varying degrees of inflammation. Despite several hypotheses regarding possible causes, including exposure to pesticides, fluoride, and heavy metals, none have yielded convincing evidence of an aetiological factor.

One of the biggest challenges in addressing CKD/CKDu is a lack of data regarding incidence, deaths, and changing trends in affected areas. While early studies reported a point prevalence of 3.2-3.7% in some divisions, recent evidence suggests a much higher prevalence of 15-23% in the Anuradhapura and Polonnaruwa districts. The study conducted by the research seeks to fill this gap by providing a comprehensive analysis of the incidence, prevalence, and trends of CKD/CKDu in the North Central Province. In this region, the disease was first identified. Through these efforts, we hope to understand better and ultimately find a way to combat this severe disease.
